This form is used to apply for a scuba diving license or certification course with Diver’s Frontiers. Please complete the details accurately so we can prepare your training, materials, and schedule in advance.

All PADI courses are conducted in clear, easy-to-understand English, and this application helps us ensure your experience is safe, smooth, and enjoyable from the very beginning.

Please complete one form per participant.

For group bookings, each person must submit their own form, but only one representative needs to list all group members’ names in “Additional Questions or Requests.”

*These are used as a reference for rental equipment

*Only fill in day 2 if you are currently on Open Water Diver. If you have already completed the Adventure Diver Course, only complete day 1. 

*If your hotel or place of residence is convenient for us, we will pick you up from that location, if not we will inform you to meet us at a specific location.

Please select your rental gear. You must choose one of the following: Full Wetsuit Rental, Full Drysuit Rental, or No Rental Needed. Full rental sets are included in the course price. If you have some of your own gear, please also select any individual items you need Required

*For the 5 Advanced Open Water dives, one Deep dive and one Navigation dive are required. If you are struggling to choose the additional dives, check out the PADI website.

You may choose an additional three Adventure Dives

We will contact you about further details on where to meet and what you need to know about the course date. Payments will be processed at the end of the dive day. Cash and Credit Cards are accepted.
